HVP – Adventist HealthCare Selects Wellcentive to Advance Population Health Management throughout its Clinically Integrated System
ATLANTA, Feb. 22, 2016 /PRNewswire/ — Wellcentive, a leader in value-based care solutions, today announced a comprehensive agreement providing population health management technology to Adventist HealthCare.
Adventist HealthCare is a faith-based, not-for-profit, integrated healthcare delivery system based in Gaithersburg, MD that includes nationally accredited acute care and specialty hospitals, mental health services and home health agencies serving the Washington, D.C., metropolitan area.
In addition, Adventist manages the Mid-Atlantic Primary Care ACO, one of the largest Medicare Shared Savings Programs serving 14,000 Medicare patients through its physician network of more than 1,000 providers in the metropolitan D.C. area. The Adventist physician team also supports the One Health Quality Alliance (OHQA), a physician-led, clinically integrated network of more than 40 physician groups.
