HVP – Wellcentive to Empower Two New Pediatric Population Health Programs at Seattle Children’s Hospital
ATLANTA, Oct. 13, 2015 /PRNewswire/ — Wellcentive, a leader in value-based care and population health management solutions, announced today that it will empower two new innovative initiatives in Pediatric Population Health Management: Seattle Children’s Care Network (SCCN) and Pediatric Partners in Care (PPIC), both at Seattle Children’s Hospital.
Through a three-year contract, Wellcentive will deliver its population health management and value-based care solutions to support SCCN, Seattle Children’s recently developed Clinically Integrated Network; and PPIC, a program funded by a $5.56 million CMS Innovation Center Grant that aims to improve health outcomes and reduce costs of care for some 3,000 disabled children with multiple medical issues.
